Group Application & Salary Deduction Forms

Group Applications are used to contract with Legal Club of America®. There are two sample group application downloads. One is for our various Legal Programs and the other is for our comprehensive Identity Theft program.

Employers fill these forms out when choosing to offer our programs to their employees. The programs can be offered on a Voluntary or Employer Paid basis.

The Application and Salary Deduction form is used to enroll employees interested in joining one of our programs. Again, a sample has been provided for your review.

Employee Communiqués

The sample communiqués are used during the employee education and enrollment process. These product sheets can be customized to meet an employers specifications and to reflect any combination of product offerings. Additionally, these forms can be transitioned on to an Employer’s web portal for online enrollments.
